Procedure to receive a medical consultation (First Visit)

* If it is your first visit to this hospital, you cannot make an appointment in advance. Please come to the hospital during the registration hours.
*Even though you have been to this hospital before, if you wish to see a doctor at a medical department that you have not attended before, you are regarded as a first visit patient.

①Reception Counter #1- New Patients Reception
- Collect a "Registration Form" and fill it out. Submit it to #1 counter with your Japanese national health insurance card if you have one.
- Please wait until your name is called from counter #1
②Medical Chart Creation
③Outpatient Department Reception Counter
- Please go to the department that was designated by the reception and wait until your name is called.
- While you are waiting, a nurse will bring you a medical questionnaire. Please answer all questions and submit it to the outpatient department reception counter.
④Consultation/Examination
Please collect a plastic folder from a nurse after consultation finishes.
⑤Payment Reception/Appointment Desk (Counter #3-#6)
- Please submit your folder to counter #3-#6.
- You will receive a payment ticket here. Please wait until your ticket number is displayed on the TV screen near Counter #6.
- If your doctor has made your appointment for the next time, the appointment slip is also issued here.
⑥Automatic Payment Machine
- You can pay by cash or credit card.
- Rental fee for crutches should be paid by CASH ONLY.
- A medication ticket is issued here if your doctor has prescribed medicine.
Please wait until your medication ticket number is displayed
on the electric bulletin board next to the pharmacy (Counter #9).
